Office Policies

Billing Policy

We participate with Blue Cross Blue Shield of Michigan, Blue Care Network, Medicare, ASR (Physicians Care Network), NGS and Priority Health. For all other insurance, payment is due at the time of service. We do not participate with Medicaid.

As a courtesy to our patients, our billing department will file a claim with your primary and secondary insurance carriers. You are expected to pay any co-pay, deductible, or non-covered charges at the time of service. Uninsured patients are expected to pay in full at the time of services as well.

For worker’s compensation cases, you need to provide us with all the necessary authorizations in writing to file your claims. You will be responsible for any services not covered.

 

Missed Appointment Policy

To continue to provide prompt, quality care, the following policy has been created for the patients of Little Traverse Primary Care:

  1. Unless there are extenuating circumstances, a 24-hour notice of cancellation is required for all scheduled appointments. Failure to give a 24-hour notice may result in a $83.00 fee for a standard office visit or $125.00 fee for a physical exam or procedure. This fee cannot be billed to an insurance company.
  2. A patient may be asked to reschedule their appointment if they are more than 5 minutes late for that appointment. A call to verify availability is appreciated.
  3. Any new patient scheduled for an appointment who did not call to cancel or reschedule will be considered a 'no show' and will be charged a fee of $125.00. This feel will need to be paid in full before another appointment can be scheduled.
  4. A first ‘no show’ appointment will result in a letter and a copy of our missed appointment policy being sent to the patient.
  5. A second ‘no show’ appointment will result in a letter and $83.00 charge being sent to the patient. The missed appointment fee cannot be billed to an insurance company.
  6. A third ‘no show’ appointment will result in a $83.00 charge and possible discharge from Little Traverse Primary Care. The missed appointment fee cannot be billed to an insurance company.


 

 

After Hours Care

If you are experiencing a medical emergency, please call "911" immediately.

Before or after regular office hours, our doctor on call can be contacted through the Northern Michigan Regional Hospital Switchboard at 231-487-4000.

If you need to be seen before or after regular office hours, we recommend the services provided at the Northern Michigan Regional Hospital Emergency Department.
